TRYOUTS BEGIN MARCH 3, 2017 for Baseball, Softball, Girls/Boys Lacrosse, Boys Tennis, Golf, Track

PHYSICAL - Current physical (within the last 365 days) completed on the NJSIAA physical form. NJSIAA Physical Form 

UPDATED HEALTH HISTORY FORM - This form must be completed each season (i.e. fall, winter, spring sport played unless physical was completed by physician within 60 days of tryouts.).     Updated Health History Form

ONLINE REGISTRATION - All athletes at Warren Hills must register online at https://warrenhills-ar.rschooltoday.com for the 2016-2017 school year AND complete the Online Usage Agreement Form.*   Online Use Agreement Form

Private Transportation Form - Athletes are required to ride the bus to and from all events.  If a student must ride home with a parent/guardian for family reasons, a Private Transportation form must be completed and returned to the Athletic Office 24 hours prior to the competition. Athletic Registration 2016-2017 Athletic Registration for 2016-2017 is now OPEN! To register your Student Athlete for fall sports and use of the weight room/open gyms, please go to our rSchool Athletic Participation site https://warrenhills-ar.rschooltoday.com or the WH Athletic Tab for the login information. In order to be eligible for try-outs, athletes must complete the online registration, submit a valid physical (performed in the last 365 days),  an Updated Health History Form, and the Online Usage Agreement Form to the Athletic Office as soon as possible.

MEDICAL FORMS: Physical Forms -  Student athletes must have a current physical (within 365 days), on file in the nurse's office in order to participate in school-sponsored interscholastic and intramural sports.  Forms are available on the rSchool registration site or from the Athletic Office.     The  PPE includes a History Form to be completed by the parent or guardian; a Supplemental History Form for Students with Special Needs; a Physical Examination Form and a Clearance Form to be completed by the examining physician, advanced practice nurse (APN) or physician assistant (PA).   NOTE: On the Clearance Form of the PPE, the last item provides a signature line for the healthcare provider to attest to completion of the Student-Athlete Cardiac Assessment Professional Development Module. The module has been developed in accordance with the law. However, it is currently undergoing adaptation to be presented as a professional continuing medical education (CME) activity and will be made available as soon as possible. You will be notified when the PD module is available.  Therefore, it is reasonable for schools to accept the student-athlete PPE without the physician, APN or PA’s attestation to completion of the PD module for this period of transition.  Schools may further reasonably expect that upon completion of the PD module, the physician, APN and PA will comply with the requirement to provide a copy of their certification to the school and will be able to sign the completion of the module attestation statement at the bottom of the Clearance Form on the PPE.  It is important for parents to understand that after the PD module becomes available, the medical home physician, APN, PA and school physician will be required to access it electronically and complete it prior to performing the medical examinations.  
 
Health History Update Questionnaire -  The parent or guardian of each student-athlete must complete and sign an updated health history questionnaire if the pre-participation physical examination was conducted 60 or more days prior to the first practice session of the athletic season. In the past, the update was required for medical examinations conducted 60 or more days prior to the start of the athletic season. The New Jersey Administrative Code will be revised to include this change required by the law. The health history update questionnaire addresses the specific questions recommended by the American Heart Association. The questionnaire must be completed and signed by the student’s parent or guardian and submitted to the school nurse.  
 
ONLINE REGISTRATION USE AGREEMENT: This form is required by hard copy ONCE A SCHOOL YEAR for your child. You may check Infinite Campus to see if we have a copy of this form on file already. An updated form will be available on July 10. 2016.
 
ACADEMIC REQUIREMENTS: ACADEMIC REQUIREMENTS need to be met for high school athletes to be eligible to participate.  The NJSIAA requires that all high school students earn 30 credits in the previous school year to be eligible for fall and winter interscholastic sports and 15 credits in the first semester of the current school year to be eligible for spring sports.  Seniors not carrying enough classes to earn 15 credits must be passing all their classes and be on track for graduation. 
 
ONLINE REGISTRATION: It will be helpful to know your child’s Student ID number and have your physician information handy to allow for accurate completion of your online registration.
