3. Provide copy of recent physical exam or note from physician
4. Complete Criminal History Review - NJ Office of Student Protection - Instructions
5. Complete the Sexual Misconduct/Child Abuse Disclosure Release
   1. Forward to previous employer to complete
   2. Or select "no applicable employment" and return form
