Substitute Security Personnel

APPLICATION INSTRUCTIONS

1. Submit completed Warren Hills Employment Application
2. Provide recent Mantoux Test (TB test) results
3. Provide copy of recent physical exam or note from physician
4. Complete Criminal History Review - NJ Office of Student Protection - Instructions
5. Complete the Sexual Misconduct/Child Abuse Disclosure Release
   1. Or select "no applicable employment" and return form
   2. Forward to previous employer to complete
