Substitute Teacher Application Instructions

1. Submit completed Warren Hills Employment Application

2. Provide Valid NJ Substitute Teacher Certificate - Instructions - https://www.nj.gov/education/certification/substitutes/index.shtml
3. Provide recent Mantoux Test (TB test) results
4. Provide copy of recent physical exam or note from physician
5. Complete Criminal History Review - NJ Office of Student Protection - Instructions
6. Complete the Sexual Misconduct/Child Abuse Disclosure Release
   1. Forward to previous employer to complete
   2. Or select "no applicable employment" and return form

Substitute Teacher Rate = $130.00/day
