Substitute Teacher Application Instructions

1. Submit completed Warren Hills Regional School District Employment Application
2. Provide Substitute Teacher Certificate - https://www.nj.gov/education/certification/substitutes/index.shtml
3. Provide recent Mantoux Test (TB test)
4. Provide recent physical exam or note from a physician
5. Complete Criminal History Review - https://www.nj.gov/education/crimhist
6. Complete the Sexual Misconduct/Child Abuse Disclosure Release
   1. Forward to previous employer to complete
   2. Or select "no applicable employment" and return form
