2025-2026 ATHLETIC CLEARANCE PROCESS
ONLINE REGISTRATION 2025-2026
Please login to the PARENT PORTAL of Genesis, go to FORMS LIBRARY and select the 2024-25 ATHLETIC REGISTRATION FORM. PLEASE sign your child up for all sports he/she may be interested in playing throughout the year to ensure you do not need to re-submit the form later in the year. All fields must be completed in order to submit the form.
If you submitted the form and your child now wishes to participate in a sport not previously registered for, go to FORMS LIBRARY and complete the “ATHLETICS-SIGN UP FOR ADDITIONAL SPORTS” FORM.
PHYSICAL CLEARANCE
PREPARTICIPATION PHYSICAL EVALUATION FORM - This form must be completed by a NJ physician who has completed the CARDIAC ASSESSMENT PROFESSIONAL DEVELOPMENT MODULE and the doctor may be asked to provide a copy of their certificate. /fs/pages/1261
The physical form is valid for 365 days.
The NJSIAA Physical Form must be completed every calendar year by a NJ Physician, Physician’s Assistant or Nurse Practitioner. Please only submit the last page of the form, THE MEDICAL ELIGIBILITY CLEARANCE FORM to the Athletic Department. (All NEW STUDENTS ie. 7th graders and students transferring from other districts must submit the entire form)
New Health History Update form
The HEALTH HISTORY UPDATE FORM is required each season/every 90 days. This form must be completed by parents/guardians and submitted to the Athletic Department prior to the start of each season.
Forms may be dropped off at the HS main office. Please note that the Cardiac Assessment Module on the Medical Eligibility form must be signed by the medical professional who completed the physical examination. The school physician is then required to review this form before any participation by the athlete.
PLEASE NOTE PHYSICAL CLEARANCE MAY TAKE UP TO 1 WEEK FROM RECEIPT OF THE FORMS AT WH, so be sure to leave enough time for clearance. Parents/Guardians will receive a letter by email once the school physician has cleared the yearly physical examination form. Login to Genesis each season to see what is needed for clearance and to check the clearance status each season.

Please submit all forms to the school nurse as early as possible to ensure participation. All forms are available in the Athletic Office or under Athletic Forms on the Athletic Page of the Warren Hills Website.
A green checkmark will appear in Genesis under your child’s photograph stating “CLEARED FOR FALL SPORTS” or if your child is not cleared, there will be a notation stating what is needed for clearance ie. Vision Screening by the eye doctor.
