FALL 2023 - MS TRYOUTS   FIELD HOCKEY & VOLLEYBALL September 5, 6 and 8th; Field Hockey will start practices 9/11, volleyball will start practices 9/12.   BOYS/GIRLS SOCCER & XC will start/hold tryouts on 9/11, 9/12 and 9/13 and will begin regular practices on 9/15.

  ONLINE REGISTRATION 2023-2024
  The Genesis online registration form will be available on July 7, 2023.  Please login to the PARENT PORTAL of Genesis, go to FORMS LIBRARY and select the 2023-24 ATHLETIC REGISTRATION FORM.  PLEASE sign your child up for all sports he/she may be interested in playing throughout the year to ensure you do not need to re-submit the form later in the year.  All fields must be completed in order to submit the form.
  
  PHYSICAL CLEARANCE
  NJ PRE-PARTICIPATION PHYSICAL FORM
  This form must be completed by a NJ physician who has completed the CARDIAC ASSESSMENT PROFESSIONAL DEVELOPMENT MODULE  and the doctor may be asked to provide a copy of their certificate.
  
  The physical form is valid for 365 days.
  
  Students with physicals that expire "in-season" are granted a 7 day grace period and may continue to participate, provided a new exam has been reviewed and cleared within the 7 days.
  
  Physicals that expire outside of season are no longer valid and the athlete may not participate until a new physical has been reviewed and cleared.
  
  NEW HEALTH HISTORY UPDATE FORM - This form is required each season.
  PLEASE NOTE, the school physician must grant final clearance for all athletes.  This process may take from 3-10 days depending on the availability of the physician and WH staff.  To ensure clearance, please begin the process EARLY!
  
  

• All forms must be hand delivered or mailed to the school nurse for review. Emails and faxed forms will not be accepted.
  
  A note will be placed on the athlete's Genesis account if the forms are incomplete.  Incomplete forms will be left in sealed envelopes in the main office of each school to be picked up by parents, finalized, and re-submitted for review.
